Overview

CAB-ULA (Cabotegravir Ultra Long-Acting) is an investigational, small-molecule, ultra-long-acting injectable formulation of cabotegravir, an HIV-1 integrase strand transfer inhibitor (INSTI). Developed by ViiV Healthcare, it is designed to extend the dosing interval to at least four months (Q4M) for both HIV treatment and pre-exposure prophylaxis (PrEP), compared to the two-month interval of the currently approved long-acting formulation. By binding to the integrase active site and blocking the strand transfer step of retroviral DNA integration, it prevents the HIV genome from incorporating into the host cell's DNA. Phase 1 data has demonstrated a pharmacokinetic profile supporting these longer intervals, and registrational studies for HIV prevention were initiated in 2024.
