CADM1-GGFG-exatecan is an antibody-drug conjugate (ADC) designed to target Cell Adhesion Molecule 1 (CADM1), a cell-surface protein identified as a promising therapeutic target in osteosarcoma. The ADC consists of a monoclonal antibody specific for CADM1 conjugated to the potent DNA topoisomerase I inhibitor exatecan via a protease-cleavable GGFG (glycyl-glycyl-phenylalanyl-glycyl) linker. This specific linker-payload configuration is optimized for efficient internalization and lysosomal processing within tumor cells. Preclinical studies in patient-derived xenograft (PDX) models of osteosarcoma have demonstrated that CADM1-GGFG-exatecan possesses superior anti-tumor activity and internalization kinetics compared to other linker variants, such as those utilizing PEG3-VC structures. It has shown the ability to induce maintained complete responses and significantly prolong event-free survival in various osteosarcoma models.
