Cadonilimab + lenvatinib is a combination therapy being investigated for the treatment of hepatocellular carcinoma (HCC), specifically as a conversion therapy for unresectable cases. Cadonilimab (AK104) is a humanized, tetravalent, bispecific antibody developed by Akeso Biopharma that simultaneously targets the PD-1 and CTLA-4 immune checkpoints. By blocking these pathways, it aims to restore and enhance the anti-tumor activity of T-cells. Lenvatinib is an oral, small-molecule multi-kinase inhibitor developed by Eisai that targets vascular endothelial growth factor receptors (VEGFR1-3), fibroblast growth factor receptors (FGFR1-4), platelet-derived growth factor receptor alpha (PDGFRα), RET, and KIT. The combination of dual checkpoint inhibition and anti-angiogenic therapy is intended to provide synergistic anti-tumor effects, potentially shrinking tumors to a size where surgical resection becomes feasible. This specific regimen is being evaluated in a Phase II clinical trial led by Peking Union Medical College Hospital.
