Cadonilimab (AK104) plus mFOLFOX6 is a combination therapy being evaluated for the neoadjuvant treatment of locally advanced rectal cancer (LARC). Cadonilimab is a humanized tetravalent IgG1 bispecific antibody developed by Akeso that simultaneously targets the immune checkpoints PD-1 (programmed cell death-1) and CTLA-4 (cytotoxic T-lymphocyte antigen-4). By blocking these pathways, it relieves immunosuppressive effects and reverses tumor-specific T cell exhaustion. mFOLFOX6 is a standard chemotherapy regimen consisting of oxaliplatin, leucovorin, and 5-fluorouracil. In clinical research led by investigator Wan He at Shenzhen People's Hospital, this combination is administered following short-course radiotherapy (SCRT) to improve the pathologic complete response (pCR) rate in patients with mismatch repair-proficient (pMMR) or microsatellite-stable (MSS) rectal cancer.
