**CAdVEC** is an investigational binary oncolytic adenovirus immunotherapy developed by Baylor College of Medicine. It combines a replication-competent oncolytic adenovirus with a non-replicating helper-dependent adenovirus that provides large transgene cargo capacity. The clinically described first-generation configuration, **CAdTrio**, locally expresses interleukin-12, a PD-L1-blocking mini-antibody, and an HSV thymidine kinase safety switch. Following intratumoral administration, the oncolytic component preferentially replicates in and lyses tumor cells, while the helper-dependent vector remodels the tumor microenvironment by promoting inflammatory immune activity, CD8-positive T-cell infiltration, IL-12-mediated immune stimulation, and relief of PD-L1-mediated immune suppression. CAdVEC is being evaluated in a phase 1 study in advanced or refractory HER2-positive solid tumors, including in combination with HER2-specific autologous CAR T cells.
