Caffeic acid is a naturally occurring small molecule phenolic compound being repurposed by The First Affiliated Hospital of Henan University of Science and Technology for the treatment of advanced esophageal squamous cell carcinoma (ESCC). It functions as an inhibitor of GASC1 (gene amplified in squamous cell carcinoma 1, also known as KDM4C or JMJD2C), a histone demethylase that is frequently overexpressed in various cancers and plays a critical role in maintaining cancer stem cell properties. By targeting GASC1, caffeic acid inhibits its expression and demethylation activity, which in turn suppresses tumor progression and reduces cancer stemness through the NOTCH1 signaling pathway. While caffeic acid is already approved in China for the treatment of thrombocytopenia, it is currently being evaluated in a Phase II multicenter trial (GiCAEC) specifically for Chinese patients with advanced ESCC who have failed prior chemotherapy or chemoradiotherapy.
