calaspargase pegol-mknl + decitabine + venetoclax is an investigational combination therapy being evaluated for the treatment of relapsed or refractory T-cell acute lymphoblastic leukemia (T-ALL) and T-cell lymphoblastic lymphoma (T-LLy). The regimen combines three distinct mechanisms: calaspargase pegol-mknl is a pegylated recombinant L-asparaginase that depletes the amino acid asparagine, which leukemic cells cannot synthesize; decitabine is a hypomethylating agent that inhibits DNA methyltransferase to reverse epigenetic silencing of tumor suppressor genes; and venetoclax is a selective inhibitor of the anti-apoptotic protein BCL-2. This triplet therapy is currently being studied in a Phase 2 clinical trial led by the M.D. Anderson Cancer Center, specifically targeting pediatric, adolescent, and young adult populations who have failed prior therapies.
