Calcium polystyrene sulfonate is a cation-exchange resin used to treat hyperkalemia (high blood potassium levels). It works by exchanging its calcium ions for potassium ions in the gut, particularly in the large intestine. The bound potassium is then excreted in the feces, thereby lowering serum potassium levels. The drug is not absorbed from the gastrointestinal tract and acts locally within the gut. Each gram of resin can theoretically exchange about 1.3 to 2 millimoles of potassium in vitro, though actual binding capacity may be lower in vivo. It is primarily indicated for patients with elevated serum potassium and should not be used if serum potassium is normal or low[3][4][6].
