CalliSpheres + arsenic trioxide (CBATO) is a drug-eluting bead (DEB) formulation used in transcatheter arterial chemoembolization (TACE) for the treatment of unresectable hepatocellular carcinoma (HCC), particularly large (5-10 cm) or huge (≥10 cm) tumors. The system utilizes CalliSpheres microspheres, which are ion-exchange hydrogel beads that serve as both an embolic agent to occlude tumor blood supply and a drug delivery vehicle. These beads are loaded with arsenic trioxide (ATO), a small molecule inorganic compound that is released locally in a sustained manner over approximately 48 hours. The therapeutic mechanism involves inducing tumor cell apoptosis and suppressing angiogenesis by inhibiting vascular endothelial growth factor (VEGF) pathways. Clinical data indicates that CBATO-TACE significantly improves progression-free survival (PFS) and overall survival (OS) compared to conventional TACE (cTACE) while reducing extrahepatic collateral artery formation and pulmonary metastasis.
