This combination therapy consists of camrelizumab (a humanized monoclonal antibody targeting PD-1), apatinib (a small-molecule tyrosine kinase inhibitor selective for VEGFR-2), and radiotherapy. It is being investigated as a first-line treatment for advanced mucosal melanoma, a rare and aggressive subtype of melanoma. Apatinib acts as an anti-angiogenic agent by inhibiting vascular endothelial growth factor receptor 2 (VEGFR-2), which blocks tumor angiogenesis and may normalize tumor vasculature to improve the delivery of other therapies. Camrelizumab functions as an immune checkpoint inhibitor, blocking the interaction between PD-1 and its ligands to restore T-cell mediated anti-tumor activity. The addition of radiotherapy is intended to induce immunogenic cell death, potentially synergizing with the systemic therapies to overcome the low response rates typically seen with immunotherapy alone in mucosal melanoma.
