Canagliflozin + insulin aspart is a combination therapy consisting of two antidiabetic agents used together to improve glycemic control in patients with diabetes mellitus, primarily type 2 diabetes. Canagliflozin is a small molecule sodium-glucose co-transporter 2 (SGLT2) inhibitor that reduces renal glucose reabsorption, thereby increasing urinary glucose excretion and lowering blood glucose levels. Insulin aspart is a rapid-acting recombinant insulin analogue that mimics endogenous postprandial insulin spikes by binding to the insulin receptor and activating downstream signaling pathways for cellular glucose uptake and metabolism. The combination targets both fasting and postprandial hyperglycemia through complementary mechanisms—canagliflozin acts independently of pancreatic beta-cell function, while insulin aspart directly replaces or supplements endogenous insulin action[1][3][6]. This regimen may be considered when monotherapy or dual oral therapy does not achieve adequate glycemic control.
