Candoxatrilat is a small molecule drug that acts as a potent and selective inhibitor of neutral endopeptidase (NEP), also known as neprilysin. It is the active metabolite of the orally administered prodrug candoxatril. By inhibiting NEP, candoxatrilat prevents the breakdown of several vasoactive peptides including atrial natriuretic peptide (ANP), brain natriuretic peptide (BNP), bradykinin, substance P, and enkephalins. This leads to increased levels of these peptides in circulation, resulting in vasodilatory and potential tissue-protective effects.
