Overview

Cangrelor + ticagrelor is a combination of two antiplatelet agents used to achieve rapid and sustained platelet inhibition in acute settings, such as during percutaneous coronary interventions (PCI), particularly in high-risk patients. **Cangrelor** is a selective, reversible, intravenous P2Y12 receptor antagonist with a nucleotide-mimetic structure, characterized by rapid onset and offset of action, leading to profound platelet inhibition within minutes. **Ticagrelor** is an oral, reversible, allosteric antagonist of the P2Y12 receptor; it binds at a non-ADP site, inducing a conformational change. When combined, cangrelor provides immediate platelet inhibition that bridges the time required for ticagrelor to reach maximal effect. Concomitant administration does not result in a clinically significant drug–drug interaction due to their distinct binding modalities[1][2][3][5][6]. This regimen is particularly relevant in ST-segment elevation myocardial infarction (STEMI) or in clinical scenarios requiring rapid transition from intravenous to oral antiplatelet therapy.
