Cannabidiol (CBD) is a non-psychoactive small molecule cannabinoid being investigated as an adjunct to massed Prolonged Exposure (mPE) therapy for the treatment of post-traumatic stress disorder (PTSD). Developed in this context by The University of Texas Health Science Center at San Antonio, the treatment regimen involves a 250mg oral dose of CBD taken twice daily alongside intensive psychotherapy sessions. CBD is hypothesized to enhance the efficacy of mPE by modulating the endocannabinoid system—specifically through 5-HT1A receptor agonism and FAAH inhibition—to reduce anxiety and facilitate extinction learning, which is the process of weakening the association between trauma cues and fear responses.
