MBX-2109 (canvuparatide) is a long-acting parathyroid hormone (PTH) peptide prodrug being developed by MBX Biosciences for the treatment of chronic hypoparathyroidism. Utilizing the proprietary Precision Endocrine Peptide (PEP) platform, MBX-2109 is designed to provide continuous, infusion-like PTH exposure through a once-weekly subcutaneous injection, aiming to reduce the daily peak-to-trough ratios seen with standard daily PTH dosing. Upon administration, the prodrug is converted into a biologically active PTH peptide agonist (PTH[1-32] with a fatty acylated Lys33) that targets the parathyroid hormone receptor to regulate calcium and phosphate homeostasis. It has received Orphan Drug Designation from the FDA and is currently undergoing Phase 2 clinical evaluation.
