CAP-1(6D) + CMV pp65 peptide-pulsed autologous dendritic cells is an investigational autologous cell-based vaccine designed to induce a targeted anti-tumor immune response against malignancies expressing carcinoembryonic antigen (CEA). The therapy involves the ex vivo maturation of a patient's own dendritic cells, which are then 'pulsed' or loaded with two specific HLA-A*0201-restricted peptides: CAP-1(6D) and CMV pp65. CAP-1(6D) is an analog of the CEA-derived epitope CAP-1 (YLSGANLNL), modified with an aspartic acid substitution at the sixth position (YLSGADLNL) to enhance MHC binding affinity and improve the activation of CEA-specific cytotoxic T lymphocytes. The CMV pp65 peptide is incorporated to act as a potent immunologic adjuvant; by recruiting pre-existing cytomegalovirus-specific memory T cells, it provides necessary inflammatory signals and help to amplify the primary response against the tumor-associated antigen CEA. This strategy aims to break immune tolerance in cancers such as colorectal and pancreatic adenocarcinoma.
