CAP4349 is an ophthalmic drug development program repurposing tafamidis, a small-molecule transthyretin stabilizer, as a topical eye drop candidate for the non-surgical treatment of cataracts. Developed by Plex Pharmaceuticals, CAP4349 is being optimized as a series of tafamidis-based prodrugs formulated to improve aqueous solubility, corneal permeation, and metabolic conversion in ocular tissues, enabling sufficient drug levels in the lens to modulate alpha-A-crystallin, a key chaperone protein responsible for maintaining lens transparency and preventing protein aggregation.[1][4][7][9][10][16] Tafamidis is already FDA approved for transthyretin-mediated cardiomyopathy, and Plex’s program leverages this known pharmacology while redirecting it toward activation and stabilization of alpha-A-crystallin chaperone activity to delay or reverse cataract formation via a convenient, non-invasive topical route.[1][4][7][9][10][16]
