Overview

This is a combination regimen consisting of three drugs—capecitabine, cetuximab, and irinotecan—used primarily in the first-line treatment of metastatic colorectal cancer (mCRC), especially in patients with RAS or KRAS wild-type tumors. - **Capecitabine** is an oral prodrug that is converted to 5-fluorouracil (5-FU) in the body and acts as an antimetabolite inhibiting DNA synthesis. - **Irinotecan** is a topoisomerase I inhibitor that prevents DNA replication and transcription by stabilizing the cleavable complex of topoisomerase I and DNA. - **Cetuximab** is a monoclonal antibody targeting the epidermal growth factor receptor (EGFR), blocking ligand binding and downstream signaling pathways involved in cell proliferation. The combination has demonstrated efficacy with objective response rates around 46–50% for KRAS wild-type mCRC patients, median progression-free survival of about 6 months, and overall survival exceeding 21 months. Toxicity profiles are manageable but include risks such as diarrhea, rash acneiform, hand-foot syndrome, vomiting, dehydration, and other chemotherapy-related adverse events[1][3][5]. The regimen may be referred to as XELIRI-cetuximab or CAPIRI-cetuximab depending on regional naming conventions.
