Overview

Capecitabine + metronidazole is not a single marketed combination drug but refers to the concurrent use of two distinct medications. Capecitabine is an oral antimetabolite chemotherapy agent used primarily for the treatment of various cancers, including colorectal, breast, gastric/esophageal/gastroesophageal junction (GEJ), and pancreatic cancer. It acts as a prodrug that is enzymatically converted to 5-fluorouracil (5-FU) preferentially in tumor tissues, where it inhibits DNA synthesis by targeting thymidylate synthase and interfering with RNA function[1][3][8]. Metronidazole is an antibiotic used to treat infections caused by anaerobic bacteria and certain protozoa[5][6]. When used together, there is a moderate drug interaction; metronidazole can increase the serum concentration of active metabolites of capecitabine (specifically 5-FU), potentially increasing toxicity risk[2][4].
