Overview

This investigational combination therapy regimen consists of the chemotherapy agents capecitabine and oxaliplatin (collectively known as CAPOX), the anti-PD-1 monoclonal antibody camrelizumab, and the anti-VEGF monoclonal antibody bevacizumab. It is being evaluated by Union Hospital, Tongji Medical College, Huazhong University of Science and Technology for the treatment of initially unresectable metastatic rectal cancer. The regimen is typically administered following short-course radiotherapy. Capecitabine acts as a cytotoxic antimetabolite prodrug of 5-fluorouracil, oxaliplatin induces DNA cross-linking to inhibit replication, camrelizumab enhances T-cell anti-tumor activity by blocking the PD-1/PD-L1 checkpoint pathway, and bevacizumab inhibits tumor angiogenesis by targeting vascular endothelial growth factor (VEGF). This multi-modal approach aims to combine the direct cytotoxic effects of chemotherapy with immune system activation and inhibition of the tumor blood supply.
