Capecitabine + oxaliplatin + PD-1 inhibitor is a neoadjuvant chemoimmunotherapy regimen primarily investigated for the treatment of locally advanced rectal cancer. This combination, often referred to as CAPOX (or XELOX) plus a PD-1 inhibitor, integrates standard-of-care chemotherapy with immune checkpoint blockade. Capecitabine is an oral prodrug of 5-fluorouracil that inhibits thymidylate synthase, while oxaliplatin is a platinum agent that induces DNA crosslinking and immunogenic cell death. The addition of a PD-1 inhibitor, such as sintilimab, aims to overcome the immunosuppressive microenvironment of proficient mismatch repair (pMMR) or microsatellite stable (MSS) tumors by restoring T-cell mediated anti-tumor activity. This triplet regimen is frequently evaluated in conjunction with short-course radiotherapy (SCRT) to improve pathological complete response (pCR) rates.
