Overview

Capecitabine + pyridoxine is a non-fixed combination of two drugs used together in clinical practice, primarily to manage side effects during cancer chemotherapy. Capecitabine is an oral prodrug of 5-fluorouracil (5-FU), a fluoropyrimidine antimetabolite that inhibits DNA synthesis by interfering with thymidylate synthase, leading to cytotoxicity in rapidly dividing tumor cells. It is indicated for the treatment of various cancers, including breast and gastrointestinal cancers[4][6]. Pyridoxine (vitamin B6) is often co-administered empirically to prevent or treat hand–foot syndrome (HFS), a common adverse effect of capecitabine therapy characterized by redness, swelling, and pain on the palms and soles[1][3]. However, multiple randomized controlled trials have shown that pyridoxine does not significantly reduce the incidence or severity of HFS when given concurrently with capecitabine[2][5][8]. The mechanism by which pyridoxine might affect HFS remains unclear but may involve modulation of skin repair pathways; its use for this indication remains controversial due to lack of proven efficacy[3].
