Overview

Capecitabine + venetoclax is an investigational combination therapy consisting of two oral small molecule drugs used in oncology. Capecitabine is an antimetabolite prodrug that is converted to 5-fluorouracil (5-FU) in the body, where it inhibits thymidylate synthase, interfering with DNA synthesis and leading to cell death, particularly in rapidly dividing cancer cells[7]. Venetoclax is a selective inhibitor of the anti-apoptotic protein B-cell lymphoma 2 (BCL-2), promoting apoptosis in cancer cells dependent on BCL-2 for survival. This combination has been studied primarily for hormone receptor-positive (HR+), HER2-negative metastatic breast cancer[1][3]. The rationale for combining these agents lies in their complementary mechanisms—capecitabine induces cytotoxic stress while venetoclax enhances apoptotic response by blocking BCL-2-mediated survival pathways.
