This combination therapy consists of capivasertib, a pan-AKT serine/threonine kinase inhibitor, and osimertinib, a third-generation irreversible epidermal growth factor receptor (EGFR) tyrosine kinase inhibitor. The combination is being evaluated in the PRECISION trial (Phase Ib/IIa), led by Shanxi Province Cancer Hospital, for the treatment of patients with advanced non-small cell lung cancer (NSCLC) harboring EGFR mutations and co-occurring PI3K/AKT/PTEN pathway alterations. Osimertinib selectively targets EGFR-TKI-sensitizing and T790M resistance mutations, while capivasertib inhibits AKT1, AKT2, and AKT3 isoforms. The rationale for this combination is to overcome resistance to osimertinib monotherapy that is driven by the activation of the PI3K/AKT signaling pathway, which serves as a common bypass mechanism in EGFR-mutated lung cancers.
