Overview

Caprylidene is a ketogenic medium-chain triglyceride (specifically caprylic triglyceride) developed as a medical food and therapeutic candidate for the treatment of mild-to-moderate Alzheimer's disease. It is metabolized in the liver to produce the ketone bodies beta-hydroxybutyrate and acetoacetate, which cross the blood-brain barrier to provide an alternative energy source for neurons with impaired glucose utilization. Originally marketed by Accera (now Cerecin) under the brand name Axona, the compound has been studied in clinical trials to improve cognitive function, showing particular efficacy in patients who lack the apolipoprotein E ε4 (APOE4) allele. While marketed as a medical food, the FDA challenged this status in 2013, leading the developer to pursue traditional pharmaceutical development pathways for related formulations like AC-1204.
