Captopril + trandolapril is a combination of two angiotensin-converting enzyme (ACE) inhibitors, both of which are small molecule drugs used primarily for the treatment of hypertension and heart failure. Both agents act by inhibiting ACE, thereby reducing the conversion of angiotensin I to angiotensin II, a potent vasoconstrictor. This leads to decreased vasoconstriction, reduced aldosterone secretion, increased plasma renin activity, and ultimately lower blood pressure. Captopril is notable for being an active drug (not a prodrug), while trandolapril is converted in vivo to its active metabolite trandolaprilat. Both drugs have demonstrated efficacy in lowering blood pressure and may also exert antiatherosclerotic effects through mechanisms related to ACE inhibition[3][5][6][7][8]. The combination would be expected to provide additive or synergistic antihypertensive effects but is not standard clinical practice due to overlapping mechanisms and potential for increased adverse effects.
