**CAR-MILs** is an autologous engineered cell therapy platform developed by WindMIL Therapeutics that uses a patient's marrow-infiltrating lymphocytes expanded from bone marrow and genetically modified to express a chimeric antigen receptor. In multiple myeloma and potentially other cancers, CAR-MILs are designed to combine the antigen-directed killing of CAR technology with the endogenous polyclonal tumor reactivity of marrow-infiltrating lymphocytes, which may provide dual tumor recognition, memory-like T-cell properties, and enhanced persistence relative to conventional peripheral-blood-derived CAR-T approaches. The platform has been described with constructs such as BCMA-directed and CD38-directed CARs and remains an investigational biologic cell therapy.
