This 4th generation CAR-T cell therapy is an investigational immunotherapy developed by The Sixth Affiliated Hospital of Wenzhou Medical University for the treatment of relapsed or refractory multiple myeloma. The therapy utilizes the 7x19 platform, which involves engineering T cells to express a Chimeric Antigen Receptor (CAR) alongside the cytokine Interleukin-7 (IL-7) and the chemokine CCL19. IL-7 is intended to support the survival and homeostatic proliferation of the CAR-T cells, while CCL19 acts to recruit host T cells and dendritic cells into the tumor site, potentially enhancing the overall anti-tumor immune response and overcoming the immunosuppressive tumor microenvironment. The clinical program evaluates various single and dual-antigen targeting strategies, including B-cell maturation antigen (BCMA), Syndecan-1 (CD138), ADP-ribosyl cyclase/cyclic ADP-ribose hydrolase 1 (CD38), SLAM family member 7 (CS1), and Integrin beta-7, to address tumor heterogeneity and prevent antigen-escape-mediated relapse.
