CAR.B7H3.CD28.4-1BB is an experimental third-generation chimeric antigen receptor (CAR) T-cell therapy designed to target B7-H3 (CD276), an immune checkpoint molecule overexpressed in various cancers, including acute myeloid leukemia (AML). The construct utilizes a novel B7-H3-specific binder and incorporates dual co-stimulatory domains, CD28 and 4-1BB, to enhance T-cell activation, persistence, and anti-leukemic potency. Preclinical studies in AML models have demonstrated that these CAR T cells effectively eradicate B7-H3-positive leukemic blasts and stem cells, showing significant survival benefits in xenograft mouse models. The therapy is being developed by a consortium led by Bambino Gesù Children's Hospital to address the therapeutic challenges of AML relapse and resistance.
