Chimeric antigen receptor 19 T cells (CAR19-T cells) are autologous T lymphocytes that have been genetically engineered to express a synthetic receptor (the chimeric antigen receptor, or CAR) that specifically targets CD19, a cell surface antigen highly expressed on B cells. Once reinfused into the patient, these modified cells recognize and bind CD19 on malignant B cells, triggering T cell activation and cytotoxicity, leading to destruction of the targeted cancer cells. This therapy represents a personalized, cell-based gene therapy and a form of immunotherapy, primarily used for the treatment of B-cell hematological malignancies such as acute lymphoblastic leukemia (ALL) and diffuse large B-cell lymphoma (DLBCL), especially in relapsed or refractory cases. CAR19-T cells are a paradigm for living drugs in oncology, and several commercial products based on this technology (e.g., tisa-cel, axi-cel) are FDA-approved. The approach was pioneered at institutions such as the University of Pennsylvania[2][5][6][1][3][4][7].
