CAR19 T cells carrying cytoplasmic activated PD-1 are a form of autologous chimeric antigen receptor (CAR) T cell therapy engineered to target the B-cell antigen CD19 while modulating the immune checkpoint protein programmed cell death protein 1 (PD-1). These modified T cells are designed to enhance anti-tumor activity by reducing or blocking inhibitory signaling through the PD-1 pathway, which is often upregulated in tumor microenvironments and leads to functional exhaustion of conventional CAR-T therapies. The approach involves either retaining or modifying the intracellular/cytoplasmic domain of PD-1 within the engineered T cell to alter its regulatory effects. This dual targeting aims to improve efficacy against refractory or relapsed B-cell lymphomas and other related hematologic malignancies[2][3][4]. The therapy is under clinical investigation for its potential benefits over standard anti-CD19 CAR-T treatments.
