Carbachol is a synthetic choline ester and potent parasympathomimetic agent that acts as an agonist at both muscarinic and nicotinic acetylcholine receptors. It is primarily used in ophthalmology to induce miosis (pupil constriction) during surgery and to reduce intraocular pressure in the treatment of glaucoma. Carbachol increases aqueous humor outflow by contracting the ciliary muscle and iris sphincter muscle. Unlike acetylcholine, it is more resistant to hydrolysis by acetylcholinesterase, resulting in a longer duration of action. Systemically absorbed carbachol can produce parasympathetic effects on various organ systems[1][2][3][4][5].
