Overview

Carbamazepine + acyclovir is a combination therapy currently under clinical investigation for the treatment of idiopathic trigeminal neuralgia. Carbamazepine, a dibenzazepine derivative, is a well-established anticonvulsant that functions as a non-selective voltage-gated sodium channel blocker, stabilizing hyperexcited nerve membranes and inhibiting repetitive neuronal firing. Acyclovir is a synthetic purine nucleoside analogue that acts as a competitive inhibitor of viral DNA polymerase, primarily targeting herpes simplex and varicella-zoster viruses. The rationale for this combination in trigeminal neuralgia stems from the hypothesis that viral reactivation within the trigeminal ganglion may contribute to the etiology or exacerbation of the condition. By combining the analgesic effects of sodium channel blockade with the antiviral activity of acyclovir, the therapy aims to provide superior pain relief and improve quality of life compared to carbamazepine monotherapy.
