Overview

This is a combination of two small molecule drugs, carbamazepine and mirtazapine. Carbamazepine is an anticonvulsant and mood stabilizer primarily used to treat epilepsy, trigeminal neuralgia, and bipolar disorder. It acts mainly by inhibiting voltage-gated sodium channels in the central nervous system, thereby reducing neuronal excitability. Mirtazapine is an antidepressant classified as a noradrenergic and specific serotonergic antidepressant (NaSSA). It works by antagonizing central presynaptic alpha-2 adrenergic inhibitory autoreceptors and heteroreceptors, increasing norepinephrine and serotonin release; it also blocks certain serotonin receptors (5-HT2 and 5-HT3) as well as histamine H1 receptors. The combination may be used off-label for treatment-resistant depression or other psychiatric indications but is not available as a fixed-dose product; rather, both drugs are prescribed together when clinically indicated.
