Carbamoylated erythropoietin is a chemically modified derivative of erythropoietin in which lysine residues are carbamoylated, resulting in loss of erythropoietic activity while retaining potent neuroprotective and tissue-protective effects. Unlike recombinant erythropoietin, it does not elevate hematocrit or blood pressure. Carbamoylated erythropoietin acts primarily via a heteromeric erythropoietin receptor/CD131 complex distinct from the homodimeric receptor involved in erythropoiesis. It has demonstrated beneficial effects on cognitive function, neuroplasticity, and renal protection in preclinical and early clinical studies, without the risk of hematologic or thrombotic complications associated with native erythropoietin. It has been investigated in clinical trials for neurodegenerative conditions such as Friedreich's ataxia, and preclinically for acute kidney injury, cognitive dysfunction in neuropsychiatric diseases, anxiety, depression, and PTSD[1][2][3][4][6][7][8].
