Overview

Carbasalate calcium is a platelet aggregation inhibitor consisting of a calcium salt of acetylsalicylic acid (aspirin) and urea. In the context of the ALPrES2MA clinical trial conducted by Medisch Spectrum Twente, it is utilized as a mono antiplatelet therapy (80mg daily) to prevent anastomotic leakage (AL) following elective colorectal surgery. The drug is administered to maintain the patency of endovascular stents placed in the superior mesenteric artery (SMA) of patients with significant stenosis and to reduce overall atherosclerotic risk. Its mechanism of action involves the irreversible inhibition of cyclooxygenase-1 (COX-1) and cyclooxygenase-2 (COX-2) enzymes, which blocks the synthesis of thromboxane A2, a potent inducer of platelet aggregation. This antiplatelet effect is intended to improve microvascular perfusion at the surgical site, thereby reducing the risk of ischemic complications like anastomotic leakage.
