Carbimazole + I-131 is a combination therapy used primarily in the management of hyperthyroidism (including Graves' disease and toxic nodular goitre). Carbimazole is an antithyroid drug that inhibits thyroid hormone synthesis by blocking the organification of iodine. Iodine-131 (I-131) is a radioactive isotope that selectively accumulates in thyroid tissue and destroys it through beta radiation. This combination may be used sequentially or adjunctively to control thyrotoxicosis before or after radioiodine ablation. The timing and sequence are clinically important because concurrent use can reduce the efficacy of radioiodine; typically, carbimazole is withdrawn several days before administering I-131 to maximize uptake, then sometimes resumed after treatment to manage symptoms until the full effect of radioiodine occurs[3][5][8]. The primary indication for this regimen is hyperthyroidism; it may also be used in some cases for thyroid carcinoma.
