Carbon-11 cholylsarcosine is a radiolabeled synthetic bile acid analog developed for use as a positron emission tomography (PET) tracer. It is composed of cholic acid conjugated with sarcosine (N-methylglycine), where the methyl group is labeled with the positron-emitting isotope carbon-11. Unlike natural bile acid conjugates like glycocholate or taurocholate, cholylsarcosine is resistant to deconjugation and dehydroxylation by intestinal bacteria, allowing it to remain intact throughout the enterohepatic circulation. This stability makes it a highly effective probe for the non-invasive quantification of hepatic bile acid uptake, biliary secretion, and intestinal reabsorption. It is primarily utilized in clinical research to investigate hepatobiliary function, cholestatic liver diseases, and bile acid malabsorption syndromes.
