Overview

The combination of carboplatin, cyclophosphamide, and mitoxantrone is a high-dose chemotherapy regimen that has been studied primarily for the treatment of metastatic breast cancer and ovarian cancer. This combination has been investigated in phase I/II clinical trials with peripheral blood stem cell transplantation (PBSCT) or autologous bone marrow transplantation (ABMT) to support hematopoietic recovery. ### Regimen Details This combination chemotherapy regimen has been administered in various dosing schedules. In one phase I/II trial for metastatic breast cancer, patients received cyclophosphamide 1.5 g/m²/day, mitoxantrone 16 mg/m²/day, and carboplatin at escalating doses from 200 to 500 mg/m²/day, all given daily for 4 days (days -6 to -3). The maximum tolerated dose of carboplatin was determined to be 400 mg/m²/day when combined with the other agents. In another study focusing on ovarian cancer, patients received mitoxantrone (75 mg/m²), carboplatin (1500 mg/m²), and cyclophosphamide (120 mg/kg), followed by autologous bone marrow transplantation. A phase I study established these maximum tolerated doses for the treatment of ovarian carcinoma. ### Mechanism of Action This regimen combines three different chemotherapeutic agents with complementary mechanisms: - **Carboplatin**: A platinum-based compound that forms DNA crosslinks, inhibiting DNA replication and transcription - **Cyclophosphamide**: An alkylating agent that adds alkyl groups to DNA bases, preventing DNA replication - **Mitoxantrone**: An anthracenedione that intercalates into DNA and inhibits topoisomerase II, preventing DNA repair
