Itraconazole is included for its ability to overcome drug resistance mechanisms—particularly by inhibiting P-glycoprotein-mediated efflux—and to enhance the efficacy of cytotoxic agents. Clinical studies have shown promising response rates and overall survival benefits in second-line settings for pancreatic cancer, with acceptable toxicity profiles[1][2][3][4]. The regimen is typically administered in cycles every two weeks.
