This is a combination regimen consisting of three agents: - **Carboplatin** is a platinum-based chemotherapy that forms DNA crosslinks, leading to apoptosis in rapidly dividing cells. - **Cetuximab** is a monoclonal antibody targeting the epidermal growth factor receptor (EGFR), inhibiting downstream signaling pathways involved in cell proliferation and survival. - **Pemetrexed** is an antifolate antineoplastic agent that inhibits multiple enzymes involved in folate metabolism and DNA synthesis, including thymidylate synthase, dihydrofolate reductase, and glycinamide ribonucleotide formyltransferase. This combination has been studied primarily for advanced or recurrent non-squamous non-small cell lung cancer (NSCLC) and head and neck cancers. The rationale for combining these drugs lies in their complementary mechanisms of action—targeting both DNA synthesis/repair (carboplatin, pemetrexed) and EGFR-mediated signaling (cetuximab). Clinical studies have evaluated this regimen with or without radiotherapy; however, the addition of cetuximab to carboplatin-pemetrexed has not consistently shown additional benefit over chemotherapy alone[3][5]. Toxicities include myelosuppression (notably neutropenia), mucositis, rash (from cetuximab), nausea/vomiting, thrombocytopenia, esophagitis when combined with radiation therapy[5][3].
