Overview

This is a combination chemotherapy regimen consisting of four cytotoxic agents—carboplatin, cyclophosphamide, doxorubicin, and thiotepa. Each drug has a distinct mechanism of action targeting cancer cells at different stages or through different pathways: - Carboplatin is a platinum-based compound that forms DNA crosslinks, inhibiting DNA synthesis and function. - Cyclophosphamide is an alkylating agent that interferes with DNA replication by forming covalent bonds with DNA. - Doxorubicin (an anthracycline) intercalates into DNA and inhibits topoisomerase II, preventing the relaxation of supercoiled DNA required for transcription and replication. - Thiotepa is an alkylating agent that also crosslinks DNA strands. This combination has been used in high-dose chemotherapy regimens for metastatic or high-risk breast cancer patients, often followed by autologous stem cell transplantation. The regimen aims to intensify cytotoxic effects against tumor cells while leveraging the non-overlapping toxicities of each component[1][2][3]. It may also be considered in other cancers such as neuroblastoma[6].
