This combination therapy consists of datopotamab deruxtecan (a TROP2-directed antibody-drug conjugate), durvalumab (an immune checkpoint inhibitor), and carboplatin (a platinum-based chemotherapy agent). Datopotamab deruxtecan combines a humanized anti-TROP2 monoclonal antibody with a potent topoisomerase I inhibitor payload (deruxtecan) connected by a tetrapeptide-based cleavable linker. Once bound to TROP2 on tumor cells, it undergoes internalization and the payload is released by lysosomal enzymes, causing DNA damage and cell death. Durvalumab is a PD-L1 inhibitor that enhances T-cell immune responses, while carboplatin is a platinum-based chemotherapy that damages DNA in cancer cells. This triple combination is being developed primarily for the treatment of advanced or metastatic non-small cell lung cancer (NSCLC) without actionable genomic alterations.
