Overview

This is a multi-agent chemotherapy combination consisting of four cytotoxic drugs—carboplatin, doxorubicin, vincristine, and irinotecan. Each agent has a distinct mechanism of action targeting cancer cells at different points in the cell cycle or through different cellular processes: - **Carboplatin** is a platinum-based compound that forms DNA crosslinks, inhibiting DNA replication and transcription. - **Doxorubicin** is an anthracycline antibiotic that intercalates into DNA and inhibits topoisomerase II, leading to DNA strand breaks. - **Vincristine** is a vinca alkaloid that binds to tubulin and inhibits microtubule formation, arresting cells in metaphase. - **Irinotecan** is a topoisomerase I inhibitor that prevents the religation of single-strand breaks in DNA during replication. This combination has been investigated primarily for pediatric solid tumors such as Wilms tumor (including diffuse anaplastic Wilms tumor) and rhabdomyosarcoma. The rationale for combining these agents lies in their complementary mechanisms of action and non-overlapping toxicity profiles. Clinical studies have evaluated this regimen (often as part of more complex protocols with additional agents) for its efficacy in improving event-free survival rates compared to historical controls[2][4][5][6].
