This is a combination regimen consisting of three drugs: carboplatin, durvalumab, and pemetrexed. Carboplatin is a platinum-based chemotherapy agent that induces DNA crosslinking and apoptosis in rapidly dividing cells. Pemetrexed is an antifolate chemotherapeutic that inhibits multiple folate-dependent enzymes involved in nucleotide synthesis, thereby blocking DNA replication and cell division. Durvalumab is a human monoclonal antibody targeting programmed death-ligand 1 (PD-L1), functioning as an immune checkpoint inhibitor to enhance anti-tumor immune responses by preventing PD-L1 from binding to its receptors on T cells. This triplet regimen has been investigated primarily for the treatment of advanced or metastatic non-small cell lung cancer (NSCLC) and pleural mesothelioma, often as first-line therapy or for patients who are not eligible for other regimens[3][4][5][6]. The combination leverages both cytotoxic effects from chemotherapy and immunomodulatory effects from durvalumab.
