This is a combination chemotherapy regimen consisting of four cytotoxic agents: carboplatin, epirubicin, etoposide, and ifosfamide. Each component has a distinct mechanism of action: - Carboplatin is a platinum-based compound that forms DNA crosslinks and inhibits DNA synthesis. - Epirubicin is an anthracycline antibiotic that intercalates into DNA and inhibits topoisomerase II. - Etoposide is a topoisomerase II inhibitor that induces double-strand breaks in DNA. - Ifosfamide is an alkylating agent that causes crosslinking of DNA strands. This combination has been studied primarily for the treatment of non-small-cell lung cancer (NSCLC) and small cell lung cancer (SCLC), particularly in high-dose regimens with autologous peripheral blood progenitor cell transplantation. The regimen may be referred to as VIP-E or VIC-E depending on whether cisplatin or carboplatin is used[1][2]. It has shown activity but also significant toxicity; its use should be carefully considered based on patient selection[1][2].
