This is a combination regimen consisting of three drugs—carboplatin, etoposide, and thalidomide—used primarily in the treatment of small cell lung cancer (SCLC). Carboplatin and etoposide are both chemotherapeutic agents; carboplatin is an alkylating agent that causes DNA crosslinking and subsequent apoptosis, while etoposide inhibits topoisomerase II, leading to DNA strand breaks. Thalidomide acts as an immunomodulatory and anti-angiogenic agent by inhibiting the formation of new blood vessels required for tumor growth. The combination has been studied in clinical trials for SCLC to assess whether adding thalidomide improves outcomes compared to chemotherapy alone. While some studies showed potential benefit in tumor response rates, large randomized trials did not demonstrate a survival advantage with the addition of thalidomide but did report increased risks such as thrombotic events and peripheral neuropathy[2][6][8].
