Overview

This is a combination chemotherapy regimen consisting of three agents: - **Carboplatin** is a platinum-based chemotherapeutic that induces DNA crosslinking and apoptosis in rapidly dividing cells. - **Gemcitabine** is a nucleoside analog (antimetabolite) that inhibits DNA synthesis by incorporation into DNA and inhibition of ribonucleotide reductase. - **Liposomal doxorubicin** (specifically pegylated liposomal formulations such as Doxil or Caelyx) is an anthracycline antibiotic encapsulated in liposomes to prolong circulation time and reduce toxicity; it intercalates into DNA and inhibits topoisomerase II, leading to cell death. This combination has been investigated primarily for the treatment of recurrent ovarian cancer, especially in patients with platinum-sensitive disease. The rationale for combining these agents lies in their complementary mechanisms of action and non-overlapping toxicity profiles. Clinical studies have evaluated various combinations—most commonly carboplatin with either gemcitabine or pegylated liposomal doxorubicin—but regimens alternating all three drugs have also been explored[7][4]. The use of the triple combination aims to maximize antitumor efficacy while managing cumulative toxicities.
