Overview

This is a combination chemotherapy regimen consisting of three cytotoxic agents—carboplatin, ifosfamide, and gemcitabine. Each drug has a distinct mechanism of action: - Carboplatin is a platinum-based compound that forms DNA crosslinks and inhibits DNA synthesis and function. - Ifosfamide is an alkylating agent that induces DNA strand breaks by forming covalent bonds with DNA. - Gemcitabine is a nucleoside analog (pyrimidine antimetabolite) that inhibits DNA synthesis by incorporation into the growing DNA strand and inhibition of ribonucleotide reductase. This combination has been explored in various solid tumors for its potential synergistic effects. While combinations such as carboplatin plus gemcitabine or ifosfamide plus gemcitabine are established regimens in cancers like non-small cell lung cancer (NSCLC), urothelial carcinoma, and ovarian cancer[1][2][3][5], the specific triple-drug regimen of carboplatin + ifosfamide + gemcitabine does not appear to be widely studied or standard; most studies use cisplatin instead of carboplatin when all three drugs are combined[5]. The rationale for combining these agents lies in their complementary mechanisms and non-overlapping toxicity profiles.
