Overview

The combination of carboplatin and leronlimab is being investigated as a treatment for metastatic triple-negative breast cancer (mTNBC). This combination therapy has shown promising preliminary results in clinical trials, particularly for patients with CCR5-positive mTNBC. Leronlimab (PRO 140) is a novel humanized IgG4 monoclonal antibody that acts as a CCR5 antagonist, binding to CCR5, a cellular receptor important in HIV infection and tumor metastases. Carboplatin is a platinum-based chemotherapy agent. In a phase 1b/2 clinical trial (NCT03838367), patients treated with leronlimab and carboplatin experienced a 72% decrease in cancer-associated macrophage-like cells (CAMLs) after 30 days of treatment. This decrease was linked to a 300% increase in mean progression-free survival and a 450% increase in 12-month overall survival. The phase 1b/2 study (NCT03838367) was designed to evaluate the combination in patients with CCR5-positive mTNBC. Phase 1b was a dose-escalation phase for leronlimab (350 mg, 525 mg, and 700 mg weekly) with a fixed dose of carboplatin (AUC 5) every 3 weeks. Phase 2 was designed as a single-arm study with 30 patients. In July 2021, the trial advanced to Phase 2, confirming leronlimab would be administered at a 700 mg dosage. The trial enrolled patients over 18, demonstrating CCR5 positivity by IHC, chemotherapy and/or checkpoint inhibitor naïve in the metastatic setting, exposed to taxane and anthracyclines in neoadjuvant/adjuvant setting, with measurable disease (RECIST 1.1), ECOG performance status of 0 or 1, and a life expectancy of at least 6 months. This combination represents a novel approach targeting the CCR5 receptor in mTNBC.
